Aged Care Quality and Safety Commission - Notification of a Material Change

The Aged Care Quality and Safety Commission has updated the Notification of a Material Change form

This form must be completed where there has been a change of circumstances that materially affects the suitability of an approved provider to be a provider of aged care, including where there has been a change in key personnel. Notification of material changes must occur within 28 days after the change occurs.

SECTION E: UPDATE KEY PERSONNEL DETAILS of the Notification of a Material Change form, now requires providers to attach the following when updating key personnel details:  

If adding a new key personnel, you are required to attach evidence that they are not a disqualified individual.  This includes:

  • A current criminal history check

  • Copy of the bankruptcy/insolvency check undertaken

  • Relevant qualifications for the role being undertaken

  • If a Registered Nurse, evidence of current registration
